Video Podcast Release Form

I grant QC Amplified and CJ Williams permission to record, use, and distribute my likeness, voice, and statements captured during my participation in the Video Podcast Booth.

I understand and agree that:

1. Usage Rights – QC Amplified has the right to use the footage, including video, audio, and images, for promotional, marketing, and commercial purposes across social media, websites, and other digital platforms.
2. No Compensation – I will not receive any monetary compensation for my participation or for the use of the content.
3. Editing & Distribution – QC Amplified may edit, reproduce, or distribute the footage as needed for promotional or commercial use.
4. Revocation – This release is irrevocable, and I waive the right to approve any final edits or uses of the content.
5. Legal Consent – I am over 18 years of age and able to consent to this release. If under 18, a parent or guardian must sign on my behalf.
